Academic Activities:
Dr. Mutter's research laboratory performs translational research on the molecular pathogenesis and diagnosis of endometrial cancer and its precursor lesions. We established molecular standards for diagnosis of premalignant endometrial disease, confirming that putative endometrial precancers were indeed the monoclonal products of genetically altered cells. Using a training set of genetically defined precancers, revised histopathologic diagnostic criteria for premalignant endometrial disease were developed as Endometrial Intraepithelial Neoplasia (“EIN”). This has been clinically validated as a predictor of future endometrial cancer risk and we currently have several projects further defining variant presentations, clinico-epidemiologic associations, and molecular changes. An additional line of investigation uses loss of function of the tumor suppressor gene PTEN as a marker for preclinical disease, which we have designated as latent precancers. These normal appearing glands contain mutations that are carried forward during progression to carcinoma, confirming lineage continuity between preclinical and clinical disease phases. We have an active program examining the long term behavior of these latent precancers, and how modifying exposures alter outcomes. Through the Gynecologic Oncology Group, a multicenter cooperative treatment group, Dr. Mutter is studying the effects of hormonal intervention on preexisting EIN.
